Rental Forms

We are always happy to welcome new clients. In order to facilitate setting up a rental account, we have posted these downloadable forms. All new clients need to complete a credit application, read and sign a rental agreement, fill out a credit card authorization and provide insurance to cover the rented equipment.

We require C.O.D. payment, guaranteed with a credit card, from all new clients but we are happy to set up an account for future rentals. Just give us a call with any questions or concerns and we will be happy to work with you.
